This form is to be used for defining repair requests to be done on Western Virginia Water Authority Vehicles by the Fleet Service Center.

Authority employees should complete the form to outline specific problems affecting performance and to note when the problem was identified.  If this is a standard request for preventive maintenance or state inspection, please check the appropriate boxes below.  

Please indicate any unusual expedited return needs related to possible upcoming standby schedule conflicts.

All vehicles needing service should be dropped off no later than 7:30am the the day of your scheduled appointment unless other arrangements have been made.
